Comprehensive Medical Plan

A PPO plan contracts with doctors and hospitals to form a network and usually provides higher benefits if you use those network providers. Network providers will usually file claims and precertify care for you. You don't need to choose a primary care physician to receive care, though if you go to non-network providers, your portion of the cost will be greater, and you may have to file claims and precertify care yourself.
